Participant Release, Waiver, & Consent Form

Welcome to CutAndSewCiety,Inc. Participants Release, Waiver, & Consent Form.

To ensure a successful and safe experience for everyone, all participants are required to read and acknowledge their understanding of the below prior to receiving confirmation of enrollment to participate in any classes/workshops provided.

1.Acknowledgment of Participation

I understand that I am voluntarily participating in a Cut & SewCiety workshop, class, event, or related activity (“Activity”). I understand that these Activities may include, but are not limited to:

  • use of sewing machines

  • hand sewing needles

  • scissors and cutting tools

  • rotary cutters

  • pins

  • irons or heat tools

  • textiles, trims, reclaimed materials, and related equipment

I acknowledge that participation may involve physical activity requiring movement of hands, arms, shoulders, back, standing, sitting, lifting, cutting, sewing, and operating equipment.

2.Assumption of Risk

I understand that participation involves inherent risks, including but not limited to:

  • cuts, punctures, scrapes, burns, or minor injuries

  • muscle strain or repetitive motion discomfort

  • allergic reaction to fabrics, dust, fibers, adhesives, dyes, or other materials

  • accidental damage to personal property

  • unforeseen injury associated with use of equipment or materials

I voluntarily assume all risks, known and unknown, associated with my participation.

I certify that I am physically able to participate and will notify the instructor if I have any condition, limitation, injury, or allergy that may impact my participation.

3.Release of Liability

In consideration for being permitted to participate in Cut & SewCiety Activities, I, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and hold harmless:

Cut & SewCiety, its founder, instructors, assistants, volunteers, partners, venue hosts, sponsors, and affiliates

from any and all claims, liabilities, demands, damages, losses, costs, or expenses arising out of or related to my participation in any Activity, including injury, illness, emotional distress, property damage, or loss, except in cases of gross negligence or willful misconduct where prohibited by law.

4. Health & Wellness Acknowledgment

I understand that Cut & SewCiety provides educational, creative, and community-centered programming. I acknowledge that no instructor, volunteer, or representative is providing medical, mental health, psychological, therapeutic, or spiritual counseling services.

I understand that participation may be emotionally reflective or community-centered in nature, but Cut & SewCiety does not diagnose, treat, or advise on physical health, mental health, or spiritual matters.

I assume full responsibility for my own physical, emotional, mental, and spiritual wellbeing before, during, and after participation.

5.Personal Responsibility & Conduct

I agree to:

  • follow all safety instructions provided by instructors or staff

  • use tools and equipment only as instructed

  • immediately report unsafe conditions or injuries

  • conduct myself respectfully toward instructors, staff, and other participants

  • respect shared equipment, studio space, and materials

Cut & SewCiety reserves the right to remove any participant whose behavior creates a safety concern or disrupts the learning environment.

6.Emergency Medical Consent

In the event of an emergency, I authorize Cut & SewCiety staff or representatives to seek emergency medical assistance on my behalf if I am unable to do so.

I understand that I am solely responsible for any resulting medical costs.

7.Advance Registration Required / No Walk-In Policy

Cut & SewCiety operates through planned, community-based and mobile programming. To ensure adequate preparation, material availability, staffing, and secured workspace, all participants are required to register through the official Cut & SewCiety website no later than fourteen (14) days prior to the scheduled class date.

Walk-in attendance is not permitted under any circumstances. No exceptions will be made.

Individuals who arrive without completing registration through the website within the required registration period will not be admitted to participate.

Advance registration is required to allow Cut & SewCiety to:

  • secure appropriate classroom or community workspace

  • prepare materials, tools, and equipment

  • confirm participant capacity

  • coordinate staffing and instruction

  • complete required participant paperwork and safety review

  • conduct participant communication and registration verification as needed

Submission of a registration request does not guarantee participation. Participation is not confirmed until registration has been reviewed and accepted by Cut & SewCiety.

Cut & SewCiety reserves the right to close registration early, maintain waitlists, deny late registrations, or cancel/reschedule classes based on capacity, safety, venue limitations, staffing, or material availability.

8.Cancellation & No-Show Policy

If a participant is unable to attend a registered workshop, the participant must notify Cut & SewCiety by email prior to the scheduled class date using the designated registration or organization email address.

Failure to provide cancellation notice by email before the class start time may be considered a no-show.

Participants who register for a class and fail to attend without prior email communication may be restricted from registering for future Cut & SewCiety classes, workshops, or programming at the discretion of the organization.

Cut & SewCiety reserves the right to enforce registration restrictions related to repeated no-shows, late cancellations, or misuse of limited program capacity.

9.Personal Responsibility & Conduct

  • conduct myself respectfully toward instructors, staff, and other participants

  • respect shared equipment, studio space, and materials

10.Right to Refuse Participation / Removal for Safety

Cut & SewCiety is committed to maintaining a safe, respectful, and inclusive learning environment for all participants, staff, volunteers, instructors, and host partners.

Cut & SewCiety reserves the right to:

  • deny registration or participation

  • refuse entry upon arrival

  • remove a participant from a class, workshop, or event without refund if applicable

when reasonably necessary to protect:

  • participant safety

  • staff safety

  • volunteer safety

  • the wellbeing of other attendees

  • organization property or equipment

  • host venue policies or operations

  • the integrity of the learning environment

Reasons may include, but are not limited to:

  • unsafe handling of equipment or tools

  • threatening, disruptive, aggressive, or inappropriate conduct

  • harassment or discrimination toward staff or participants

  • refusal to follow instructor direction or safety procedures

  • suspected intoxication or impairment

  • repeated violation of program policies

If a participant is asked to leave for safety or policy violations, Cut & SewCiety reserves the right to prohibit future registration or participation in future programming.

11.Participant Identity Verification & Registration Screening

To help maintain the safety and integrity of Cut & SewCiety programs, all participants acknowledge that registration may be subject to identity verification prior to confirmation of participation.

Cut & SewCiety reserves the right to request reasonable information or documentation to verify a participant’s identity before approving registration, confirming participation, or allowing entry into a workshop, class, or event.

Verification may include, but is not limited to:

  • government-issued photo identification

  • school identification

  • confirmation of name, email address, or phone number used during registration

  • emergency contact verification

  • parent or guardian verification for minor participants

Failure to provide requested verification information within the requested timeframe may result in delayed registration, denial of participation, or cancellation of registration.

Cut & SewCiety reserves the right to decline or cancel participation when:

  • identity cannot be reasonably verified

  • submitted registration information appears inaccurate, incomplete, or inconsistent

  • a safety concern exists for staff, volunteers, participants, instructors, or host venues

  • participation is determined to pose a risk to the program environment or operations

All participant information collected for verification purposes will be used solely for registration, safety, and operational purposes and will be handled in accordance with applicable privacy practices.

*12.Parent/Guardian Attendance & Supervision Policy

Cut & SewCiety is committed to maintaining a safe and supportive environment for youth participants.

Unless a class is provided in direct collaboration with a school, educational institution, or youth-serving partner where school staff, administrators, or designated partner representatives are present and responsible for student supervision, Cut & SewCiety reserves the right, at its sole discretion, to require a parent or legal guardian to:

  • remain onsite during the duration of the class

  • remain inside the classroom or designated program space during instruction

  • remain immediately available for the duration of the Activity

Cut & SewCiety may require parent/guardian presence based on factors including but not limited to:

  • participant age

  • participant experience level

  • behavioral or support needs

  • nature of the equipment being used

  • venue requirements

  • staffing availability

  • class size

  • general safety considerations

Parents/guardians will be informed of attendance requirements in advance whenever possible.

*13.Parent Drop-Off & Pick-Up Policy

Cut & SewCiety does not guarantee drop-off childcare services unless expressly stated in writing.

Parents/guardians may not leave a minor participant unattended unless prior written approval has been communicated by Cut & SewCiety.

In the event written consent is provided the parent/guardian listed on this form agrees to:

  • arrive on time for drop-off

  • arrive promptly at the scheduled class end time for pickup

  • remain reachable by phone during the Activity

Repeated late pickups, unauthorized drop-offs, or failure to comply with supervision requirements may result in:

  • immediate contact to return to the site

  • removal of the participant from the current class

  • denial of participation in future Cut & SewCiety programming
